order (whichever method you choose), please provide the following information:

1. Name of primary user (this person will receive enduser and update/upgrade information).
2. Company name, if applicable.
3. Mailing address (street, city, state, zip).
4. FedEx address, if different (Federal Express will not deliver to p.o. box addresses).
5. Phone number.
6. Fax number, if you have one.
7. E-mail address, if you have one.
8. Full payment is required before software is shipped. We accept MasterCard and VISA. See Pricing, Rental, and Shipping page for additional information.
9. For security reasons, please do not send your credit card information by email.
