Use this form to participate as a vendor, ordering one or two tables in the exhibition, or to participate as a Silver- or Gold-level sponsor of the conference. Each vendor or sponsor receives one complementary registration, which includes the continental breakfast, luncheon, banquet, and breaks on October 1, and the continental breakfast and breaks on October 2, as well as attendance during the technical sessions.
Option to Pay by Check
Note: If any professionals, students, exhibitors, or sponsors would rather pay directly by a check through the mail rather than by credit card through the online registration form, this can be made out to "Midwest Ground Water Conference" and sent to the Conference Coordinator, Donald Whittemore, Kansas Geological Survey, 1930 Constant Ave., Lawrence, KS 66047.
